Registered Nurse Care Coordinator and Advocate

24 hours per week (may increase with time)
$33-35/hour depending on experience

Responsibilities:

  • Partner with patients to help them advocate within larger health care systems, insurance providers
  • Assist with referrals and coordination of care, especially for gender-related services
  • Assist with patient visits, vital signs, taking medical history and coordinating care plans
  • Assure timely refills via medication protocols
  • Work with the clinical team to develop and improve clinical systems and quality of care
  • Coordinate preventive care and assure appropriate follow-up
  • Perform other duties as assigned

Qualifications:

  • One year of experience in outpatient clinical setting, preferred
  • Non-judgmental attitude in caring for all humans, but especially those struggling with mental health issues or substances
  • Excellent communication skills—with clients and with the broader public
  • Detail oriented
  • Experience working with diverse populations, preferred
  • Commitment to the mission of Out Health

Certification
Valid Registered Nurse Licensure in Wisconsin

Volunteer Coordinator/Office Manager

20 hours per week (may increase with time)
$15-18/hour depending on experience

Responsibilities:

  • Partner with members of the clinical team to create a positive atmosphere that supports patient care and advocacy
  • Assist with front office functions including patient relations, check-in/check-out, scheduling, insurance verification, and answering phones
  • Process invoices, expenses, etc.
  • Monitor, order, and stock office and non-medical supplies
  • Recruit, schedule, and coordinate volunteers
  • Maintain the donor database and administer donor acknowledgements
  • Maintain confidentiality of patient and donor information
  • Assist with social media and website engagement
  • Perform other duties as assigned

Qualifications:

  • 1 year of experience in an administrative /office position, preferably in a nonprofit and/or outpatient medical/dental setting
  • Ability to perform administrative/office coordination duties at a level normally acquired through completion of one year of post high school, business or office management program
  • Ability to multitask and prioritize
  • Strong organizational skills.
  • Proficient in Microsoft Office Suite
  • Effective communication and interpersonal skills
  • Experience working with diverse populations, preferred
